Client Requirements:

Picture Identification: Driver’s License or State ID Card

Proof of Residency: Current Utility Bill (gas, telephone, electric, cable) OR Lease,

Proof of Household Size: Birth certificates OR Mass Health Cards for everyone

Proof of Income: Proof of income is needed for everyone in the household 18 years or older

- 4 consecutive weekly paystubs (2 consecutive bi-weekly paystubs)

- Social Security, SSI, TAFDC current monthly statement

- Unemployment Documentation

(Income must be within 30 days)
